Background: Loneliness is a common mental health problem in older population. Previous studies have shown that loneliness can affect older persons’ physical and mental health and increase mortality rates, and has become a serious public health issue. Therefore, understanding loneliness in older people is necessary, but there is little research on loneliness among older adults in Taiwan.
Aims: There are three aims in this series of studies, including: (1) to understand the prevalence and associated factors of loneliness among older people in Taiwan, (2) to explore the perceptions of loneliness among older residents in long-term care facilities (LTCFs), and (3) to evaluate the effectiveness of tactile massage in improving mental health of older residents in LTCFs.
Methods: Study 1: Data from the Taiwan Longitudinal Study of Aging collected in 2015 were analyzed and involved 4588 participants aged ≥65 years. Chi-square tests and multiple logistic regression were used to analyze the data. Study 2: A qualitative research design and purposive sampling was used. Sixteen older residents were interviewed in two LTCFs in Northern Taiwan using a semi-structured interview guide. Content analysis was used to analyze the data. Study 3: A randomized controlled clinical trial and convenience sampling was applied. Fifty-five older residents in three LTCFs were randomly assigned to the intervention group and comparison group. The intervention group received tactile massage for both hands twice a week for four weeks, and the control group received regular care. Data were collected before and after the intervention. Generalized Estimation Equations (GEE) was used to evaluate the effectiveness of the intervention.
Results: Study 1: The prevalence of loneliness among older adults in Taiwan was 10.5%. Old persons who were male, lived alone, perceived that they had poor health condition, had no spouse, had no job, and had poor emotional support felt greater loneliness. Study 2: Four themes were generated including being cut off from continually meaningful relationships, experiencing tears of pain, feeling alone, and lacking a sense of belonging. Study 3: There were no differences in loneliness, anxiety, depression, happiness, body temperature, heart rate, and diastolic blood pressure between the two groups before and after the intervention. However, the intervention group had significantly improved respiration rate, systolic blood pressure, comfort level, and relaxation level.
Conclusions: The one-tenth of older adults in Taiwan had loneliness. Healthcare professionals should pay more attention to older adults who are male, have no spouse, have poor self-reported health, live alone, are unemployed, and have poor emotional support in order to identify problems early and develop interventions to prevent or reduce the loneliness. In addition, healthcare professionals should assist older residents in maintaining relationships with their families, provide accompaniment, and promote a sense of belonging for them. . Moreover, tactile massage can effectively improve respiration rate, systolic blood pressure, comfort, and relaxation level in older residents. It is recommended that the healthcare professionals in LTCFs can promote the use of low-cost, easy-to-use, and easily accessible tactile massage in the future.
Healthcare professionals should assist older residents in maintaining relationships with their families, provide accompaniment, and promote a sense of belonging for them.
