D型人格(Type D personality,distressed personality)被證實可有效預測冠狀動脈心臟病(Coronary artery heart Disease,CAD)患者心臟危險事件(major adverse cardiac events,MACE )的發生。而 D型人格量表(Type D Scale-14,DS14) (Denollet, 2005) 則為目前最被接受且廣為使用之篩選D型人格工具,測量了負向情感(negative affectivity,NA)及社交抑制(social inhibition,SI )兩構成D型人格的特質。鑑於篩選具D型人格之CAD患者以進行後續臨床心理介入有其重要性,Weng、Long、Pedersen、Lin 與 Lin(2009)將DS14翻譯成中文,使用於台灣CAD樣本中,並檢驗其信效度。結果顯示,SI分量表之內部一致性及再測信度較低。為此,本研究目的在重新選題修訂台灣版DS14之信度,並將量表應用範圍擴展至CAD患者及一般成人。首先,本研究由DS24(Denollet, 2000)中選取3題信效度均佳之屬於SI題項,新增至台灣版DS14中成為共17題的D型人格量表預試題本。共83名CAD患者及413位接受健檢之一般成人,填寫了D型人格量表預試題本、中文版特質焦慮量表、中文版貝克憂鬱量表第二版及短式華人題意量表。而另一56位大學生樣本,填寫了D型人格量表預試題本,並進行兩周後再測。結果顯示,DS14原量表第3題「我會跟陌生人說話」因與SI分量表相關低,而由新增之SI題項「我不喜歡跟很多人在一起」代替。而修訂後之台灣版DS14(Type D Scale-14,Taiwanese version –revised,DS14-TR)具良好的內部一致性信度(NA= .86,SI= .80,全量表= .87),再測信度(NA= .82,SI= .88,
全量表= .86)。因素分析結果顯示量表中所有題項皆落於其所屬因子,而其效標關聯效度檢驗結果與過去研究一致: 相較於非D型人格參與者,D型人格參與者之憂鬱、焦慮與敵意分數較高。而D型人格參與者除有較多的負向情感、較高之社交壓抑傾向外,亦有較高的敵意認知。綜合上述,DS14-TR在台灣CAD與一般成人樣本中具良好的信效度,可應用於台灣臨床及研究領域。
