研究背景:面對軍中社會環境的壓力,役男如何運用因應壓力策略與其身心適應息息相關,依據國防部統計資料顯示在接受軍中諮商輔導的役男中有22%為適應不良患者,實為值得高度關注的心理健康議題。
研究目的︰本研究主要目的在探討以軍中適應症患者為對象,探討家庭功能、父母教養態度與壓力因應策略和適應障礙症患者之焦慮及憂鬱症狀的相關性,以為早期介入與治療之參考。
研究方法︰本研究在實驗性研究架構下,取樣方式是將潛在個案經電腦程式隨機分派,於潛在個案同意參與後進行收案,收案對象北部四家軍方醫院,以診斷為適應障礙之軍人及主要照顧者為研究對象,收案時間為民國102年4月1日至104年 3月31日。符合收案條件且願意參與本研究共有94組(病人與家屬為一組),個案完成研究同意書簽署後進行各量表之評估,將所得資料分析方法以描述性統計、皮爾森積差相關分析及線性複迴歸進行分析,了解各項因素對壓力因應策略的相關影響情形。
結果︰本研究經由統計分析後,其結果顯示適應障礙症軍人病患傾向使用較為負向的壓力因應策略、適應障礙症軍人病患的家庭功能傾向不良、適應障礙症軍人病患的雙親教養態度傾向低關懷高保護、家庭功能較差的適應障礙症軍人病患傾向使用較多的負向壓力因應策略
、雙親教養態度屬於低關懷高保護的適應障礙症軍人病患傾向使用較多的負向壓力因應策略及使用較多負向壓力因應策略的適應障礙症軍人病患會呈現較高的焦慮及憂鬱症狀。
結論︰此研究結果提供臨床醫療人員實務運用之參考依據,從結果分析中可發現在正向因應策略中的「正重新解釋和增長」與焦慮及憂鬱情緒呈負相關,故建議於臨床在治療適應障礙軍人病患時除了藥物治療外,可運用相關認知治療方法,協助病人面對壓力源時,如何採取正向的因應策略去因應壓力,進而可有效幫助病人降低於療程中因面對壓力源時所產生之焦慮及憂鬱情緒。
Research background: How draftees employ stress coping strategies in a stressful social environment in the army is closely connected with the physical and psychological adaptation of the draftees. Statistics released by the Ministry of National Defense, Republic of China, show that 22% of the draftees having received counseling in the army exhibited maladaptation. This is a mental health issue necessitating considerable attention.
Objectives: By focusing on patients exhibiting maladaptation in the army, this study investigated the correlations that family functions, parents’ parenting attitudes, and stress coping strategies adopted have with the anxiety and depression symptoms that patients with adjustment disorder exhibit. The results can serve as a reference for early intervention and treatment.
Methods: Under an experimental research framework, we conducted random sampling by using a computer program to randomly select potential cases. We recruited patients as participants of this study after receiving their consent. The participants were military personnel that were diagnosed with adjustment disorder and were treated at four armed forces hospitals in northern Taiwan. The primary caregivers of these people were also recruited as the participants. The participants were recruited between April 1, 2013 and March 31, 2015. Ninety-four pairs of participants (patient + family) that satisfied the inclusion criteria agreed to participate in this study. After signing research consent forms, the participants were evaluated using various scales. The data were analyzed using descriptive statistics, Pearson product moment correlation analysis, and multiple linear regression. The results were employed to understand the correlations between various factors and stress coping strategies.
Results: The results of a statistical analysis showed that military patients with adjustment disorder tended to adopt negative stress coping strategies, the family functions of military patients with adjustment disorder were ineffective, and that the parenting attitudes of the parents of military patients with adjustment disorder tended to be uncaring yet highly protective. In addition, military patients with adjustment disorder whose family functions were ineffective tended to adopt negative stress coping strategies more frequently; military patients with adjustment disorder whose parents adopted an uncaring yet highly protective parenting attitude tended to adopt negative stress coping strategies more frequently; and military patients with adjustment disorder who adopted negative stress coping strategies more frequently exhibited severer symptoms of anxiety and depression.
Conclusion: The results of this study can serve as a reference for clinical medical personnel to adopt in practice. The results indicated that “positively reinterpret and grow,” one of the positive coping strategies, was negatively correlated with anxiety and depression. Therefore, we recommend that in addition to medical treatment, the clinical treatment of military patients with adjustment disorder can incorporate relevant cognitive treatment methods. By enabling patients to adopt positive stress coping strategies in response to stressors, medical personnel can effectively assist patients in reducing the anxiety and depression that patients experience when confronted with stressors during treatment.
