出院準備服務為各家醫院的重大政策之一，更是醫療團隊必須執行的工作，透過良好的評估工具，可以將整合後的相關資源提供給個案及家屬，為出院後的照顧作更完善的準備。但在台灣地區並無以個案及家屬為主的評估表。本研究主要目的在於檢測澳洲發展使用的PREPARED量表，於台灣地區出院準備服務個案及家屬其信、效度，以了解其合適性。研究設計先將量表透過兩段式翻譯後，最後由原作者本人確定與原量表語言上之正確性，再以立意取樣方式，將出院準備服務個案及家屬作為研究對象。研究工具為PREPARED量表及SF-36，有效樣本為120位，研究後發現有良好的效度指數其值為0.86，經過因素分析後，三個共同因素可以解釋八測量題項變異達66.05％。總量表的內在一致性Cronbach’s α為.66，再測信度之積差相關係數為.895。所以PREPARED量表為具良好信、效度的工具，尤其是透過開放式的問答，更能夠讓個案及家屬充份表達心中的需求及想法。建議將PREPARED量表做為臨床醫護人員的評估工具，能夠更清楚了解及提供更適當的個案及家屬需求的服務。

Discharge planning is an important strategy that can help healthcare worker to evaluate patients’ needs and problems before discharge. PREPARED questionnaire which was developed in a Australian hospital is an evolutional tool to integrate different resources for patients. There is no such kind of integrated resources and evaluating tools in Taiwan. The propose of this study try to examine the feasibility and adaption of PREPARED in Taiwan.
In the method of study, at first, double-translation method was used to confirm PREPARED and also from original designer. Then, the data collection from patients and their families by PREPARED and SF-36 scale were correlated by statistics. There are 120 persons involved in the study.
The results: The CVI(Content validity indeed,CVI) is 0.86. With the analysis of Variance, three cofactors could explain the whole concept with variance rate 66.05%. Cronbach’s α is 0.66 and test-retest reliability is 0.895. PREPARED questionnaire is a good evaluation tool using open-question to represent patient and family’s views which get very rich qualitative data. From the results of study, the research suggests that PREPARED can use in clinical evaluation to help healthcare worker to make more feasible and appropriate discharge planning to evaluate patient’s and family’s need.
